Case 1: This case involves a 2-month-old patient whose family has several medical issues arise prior to patient’s birth. Patient’s father was temporarily unable to work due to a work-related injury, which did not offer worker’s compensation. Patient’s father was able to return to work for a few days when the patient’s grandfather suffered a severe medical injury, which resulted in hospitalization. The next day, patient’s mother went into preterm labor at 24weeks. All of these unfortunate events resulted in incurring hospital expenses and loss of income as well as dealing with lack of transportation (vehicle in shop). Family has not been able to visit patient regularly and are relying heavily on their phones to stay in touch with the medical care regarding patient’s care. It is important that the family maintain electric/power so they can keep their phones charged as well as maintain a habitable environment for their family which including an older sibling. Patient is not anticipated to discharge anytime soon due to their medical issues. Patient’s father is working 6 days a week to make up for lost income but family is trying to get back on their feet and needing assistance with their electrical bill. Case 2: This case involves a near five-month old (former preterm) patient who was transferred to ACH for treatment of prematurity and respiratory distress. Throughout patient’s hospitalization, patient has several infections. Patient is at a point in the journey where they are working on learning how to take oral feeds. Patient has remained in NICU for the entirety of their life and is looking at possible discharge in the next several weeks. Patient’s family, which includes a special needs sibling have remained at the RMH so they could remain close and learn to properly care for the baby. Prior to patient’s birth, the family’s living situation was not a healthy one and family soon decided they did not want to return to where they were living. Patient’s father found employment in the city and was working for most of patient’s stay here but was recently let go from their employer. Patient’s father was going to a job interview when sadly hit by a car and has been working on his own recovery and getting back onto the job hunt. While patient’s father has been dealing with these issues, mother has been working to find housing and establishing services for the family in the Pulaski County area. The family just got a new apartment, which is $650 a month. While family is working to get back on their feet, they are needing some assistance with the rent. We are submitting for your consideration, a request to help family with 3 months of rent payments. Total ask: $1950. APPROVED
Case 3: This case involves 17yr burn patient. Patient initially discharged last month. The plan was for the patient to continue outpatient occupational therapy however, family’s vehicle has broken down which resulted in great difficulties taking patient to appointments and therefore patient was readmitted due to significant hyper lordosis, thoracic kyphosis among other things. Reliable transportation is needed to ensure that the patient can get to outpatient OT and other follow up appointments so they can receive proper medical care. Therefore, we are submitting for your consideration, a request to help family with $1,500 towards the cost of vehicle repair. Total ask: $1,500 APPROVED
Case 4: This case involves 2-month-old patient admitted to ACH due to significant bowel distention. Patient’s father is the primary income source for family due to the limited family support available. Father has struggled to both balance work and being here with patient’s mother and patient. They family also experienced loss of family members during patient’s admission. Medical providers are anticipating a long stay due to the severity of the medical condition. This request is being submitted due to the severity of the diagnosis, immediate admission of patient and parents need to be present and available. This request would give parents the freedom to allocate finances and to help alleviate at least some of their stressors including housing and transportation. We are submitting for your consideration, a request to help family with car payment of $520.81. Total ask $520.81 APPROVED
Case 5: Patient is a 16 y/o female admitted on 4/23/24 and potential discharge in about 4 weeks. They were previously healthy until admitted following an episode of HUS (hemolytic-uremic syndrome) which is a blood disorder which attacks the blood vessels and causes damage to the kidneys. Most commonly caused by an infection from E. Coli. In their case, this caused a brain injury which severely impacted their general functioning including the ability to speak and ambulate. This has also led to a requirement of dialysis treatments three times per week due to loss of kidney function. Their mother has been at bedside throughout this admission which has caused loss of her job. Mom has worked with other family members and a Go Fund Me to take care of financial needs and has been fortunate up to this support. As we prepare for discharge, they will likely continue to be wheelchair-bound for the foreseeable future. The mother needs help with adapting to the needs of her daughter, who uses a wheelchair. She asks for $2,000 toward accessible transportation and home. The family will continue to need to commute from Prairie Grove to Little Rock, about 117 miles, a least once a month for dialysis follow up care.. Patient's mother is currently dependent upon child support from the patient's father for financial support but has also applied for SSI and is awaiting approval. Total ask $2,000 APPROVED
