Case # 1:  This case involves a teenager involved in a motor vehicle collision, which resulted in the patient suffering a traumatic brain injury and spinal fracture.  Due to the patient’s length of hospitalization, the patient’s primary financial caregiver lost their job.  This has resulted in the family becoming behind on their bills including their rent ($498) and electricity ($239.91).  Therefore, we are submitting for your consideration at total financial assistance request of $737.91  Total ask:  $737.91  APPROVED

Case #2: This case involves a complicated admission for a patient diagnosed with HUS, which causes significant impairments in kidney function, which will require dialysis treatment multiple times a week.  However in this patient’s case, this also caused a brain injury which as impacted their ability to speak and ambulate.   Patient will likely be wheelchair bound for the foreseeable future.   Patient’s mother has been at bedside throughout the admission, which led to the loss of her job.  As the patient gets closer to discharge, the family will need to plan for the frequent drives to and from Little Rock for at least monthly dialysis treatment.  Due to the wheelchair needs for this patient, the family is seeking to purchase a wheelchair assistance van to bring patient to and from treatment.  Family has been seeking financial assistance for the vehicle needs through grants, family etc.   Therefore, we are submitting for your consideration a total financial request of $3,000 towards the purchase of lift.                             TOTAL ask:  $3, 000  APPROVED

Case #3:  This case involves a patient with a complex medical history including cerebral palsy, epilepsy, and oxygen dependency.  At the time of the request, patient was hospitalized but recently passed away.  The patient lived at home with their parents who have limited resources due to being a one-income household.  Patient’s mother was the fulltime caregiver for patient as well as patient’s siblings.  Due to the patient’s passing, the family is now planning a funeral for the patient.  Due to their limited income, family has to utilize funds that normally go towards their housing needs.   Therefore, we are submitting for your consideration a request of $1100 towards the family monthly rent payment.  TOTAL request:  $1,100  APPROVED

Case #4:  This case involves a long term admitted patient due to medical complex issues including bronchopulmonary dysplasia, pulmonary hypertension and tracheostomy dependency.  Due to patient’s condition, they will need have a home living space where patient can be near a window when the patient discharges.  Patient’s caregiver recently a home but will need water lines and plumbing addressed before it is livable for patient.  Therefore, we are submitting for your consideration a request of $600 towards the down payment of the plumbing repairs.  Total ask:  $600  APPROVED

Case #5:  This case involves a teenager who have sickle cell disease.  Patient is supported by a single caregiver household with four other children.  Caregiver works three jobs and is just over the limit to receive any government assistance.  The patient has had multiple admissions due to sickle cell pain crises since this spring.  This has caused additional strain on the family and caregiver has to be at bedside with patient.  This has resulted in family getting behind on rent, utilities and having food insecurity.   Family is seeking assistance in paying rent and several utilities to help them get back on their feet.   Therefore, we are submitting for your consideration a request of $1018.76 towards rent and utilities.  Total ask:  $1018.76  APPROVED
