 Case # 1.   This case involves an ACH patient originally admitted for an abdominal injury and later found to have ruptured bowel with secondary peritonitis that required surgery.  Patient has experienced multiple complications including having an open wound that requires frequent surgeries.  These situations have dramatically changed the patient’s life.  Patient had been a competitive athlete and now due to these medical needs, patient will be dealing with chronic health issues for a very long time.  The parents both had well-paying jobs prior to this incident.  Patient’s mother lost her job with her FMLA ran out so they will be facing major life changes in their family, which includes financial requirements.  Family is seeking assistance at this time to help with their rent needs.  For your consideration, we are requesting assistance with rent for $2,400.   Total ask:  $2,400

Case #2.  This case involves a m 29 week gestation preterm infant admitted to the NICU for hypoxic ischemic encephalopathy after mom was sent for an emergency C-section.    Due to the emergent birth and mother’s hospitalization, the family did not have the time to plan ahead nor expect a long-term hospitalization for the patient.  Patient’s father is the primary source of income for the family and it has become difficult for the family to maintain financial requirements.  Father’s employer does not offer paternity leave and had very little PTO prior to the patient’s birth.  This PTO was utilized during the first week when mother was sent in for the C-section. Due to the additional care needs for both mom to recuperate at home while patient received care at ACH, father has been the primary bedside caregiver for patient.   As a result, the father has missed work, which set the family significantly behind on their monthly bills with past due rent payment for this month. They have received a warning of eviction.    Fortunately, the patient has been  discharged recently and the father will be getting back to work next week.  Family is seeking assistance with their rental needs so they can get back on their feet.     For your consideration, we are requesting assistance with rent for $1,250. Total ask:  $1,250

Case # 3.  This case involves a 2-year-old ACH Northwest patient diagnosed with biliary atresia.  Patient was transferred to an out of state hospital 6 hours away from ACNW to receive an organ transplant.  This medical condition requires patients to attend out of state follow up visits every 6 months.  Medicaid transportation and the reimbursement program were explored but the company will not provide resources for over 50 miles past the state lime, regardless of reason or medical necessity.  The parents have expressed a lot of anxiety about financial strain.  The patient’s father does temporary work, which does not provide consistent income to the family as it is based on availability.  Additionally, while patient had the surgery, the mother was in the hospital giving birth to patient’s sibling.  Mom is unable to work due to having an infant and caring for patient’s medical needs as patient requires g-tube feedings.  Patient is also at high risk for complications and organ rejection.  This requires regular medical monitoring and follow up appointments with several clinics/specialty teams.  The family is having difficulties making ends meet and having funds to make the biannual trips to the outside hospital.  Family is seeking financial assistance with gas to make these out of state appointments.  .     For your consideration, we are requesting financial assistance of $300 for gas cards.   Total ask:  $300

Case #4  This case involves a 4-month-old 29 week gestation preterm patient with chronic lung disease and difficulty tolerating feeds.  The patient has already had multiple admissions and follow up appointment to address these issues.  Patient’s father recently lost his job and has been making efforts to find employment. However, unexpected admissions has caused further financial difficulty.  The family has fallen behind on rent and are in danger of being evicted.  The family is seeking assistance with their rent needs.  For your consideration, we are requesting assistance with rent for $1,725 Total ask:  $1,725

The Committee agreed to support all of the above requests at the level requested.
