Case # 1. This case involves a 10-year-old patient currently on their fourth admission this year due to chronic Cystic Fibrosis issues. The family lives almost three hours away, which makes it difficult to travel back and forth for work, but she has to miss several work shifts due to patient’s admission needs as well as outpatient appointments. These situations have taken a toll on the family’s ability to maintain their monthly bills/expenses with a possible car repossession. For your consideration, we are seeking the following assistance to help get the family caught up: $1100 (Rent), $400 (Electric), $500 (Gas), $71.78 (Water), $300 (phone) $444 (car payment) and $175.41 (car insurance). Total ask is: $2991.19

Case # 2. This case involves a 5-year-old diabetic patient who has had multiple admissions this year and frequent outpatient appointments to maintain their regimen. Given the patient’s medical issues, the caregiver anticipates future admissions. The family’s primary caregiver and sole financial support has five children in total. The caregiver currently works but has had to miss work due to recent hospital admissions. Remaining at the bedside with the patient has caused them to miss multiple pay periods. As a result, the caregiver is behind on rent and utility bills and may face eviction and possible shut-off notices. Therefore, we are submitting for your consideration the following assistance to help the family catch up on their bills: $994 (rent) and $187.41 (Electric). Total Ask: $1,181.41

Case #3. This case involves a teenager who suffered a gunshot wound in a domestic violence situation. The patient was hospitalized for about a month, recuperating from multiple surgeries. The patient will have to have physical therapy and numerous specialty appointments moving forward. Due to the admission, the patient’s caregiver (single parent) had to stay at the bedside and was therefore unable to work, which resulted in lost wages and inability to pay their electric and rent bills. Therefore, we are submitting for your consideration the following assistance: $1,123.49 (Rent) and $800.41 (Water). Total ask: $1923.90

Case 4: This case involves a 29-week-gestation patient who was transferred to our NICU and is currently still in the hospital. We expect the patient to be admitted for several more weeks. The patient has several medical issues, including high ventilator support, shunt placement, an ostomy, and a G-tube for feeding. Due to all of these complex needs, the patient’s primary financial caregiver has had to be present for training for eventual home care. The family also has multiple siblings at home, including two adult siblings who are trying to find employment to help support the family. This has caused the family to fall behind in their rent for several months, owing over $3000. The property owner has been very compassionate about the family’s situation but notes they can no longer remain in the home if they cannot pay the back rent. Therefore, we are requesting your consideration and assistance of $3,000 towards rent.
Case 5: This case involves a 7-year-old Cystic Fibrosis patient who is currently admitted with an expected additional 3 weeks stay. This is the patient’s third hospitalization this year. The family lives about three hours away, and the caregiver must remain at the bedside during these admissions, which has impacted the family's income, causing them to fall behind on their rent and car payments. The caregiver has exhausted all of their sick time at their job during the last admission, so they will not have any income this month. The family is concerned about their vehicle being repossessed and possible eviction. Therefore, we request your consideration for the following assistance: $1395 (rent) and $420 (car payment). Total ask $1815
