Case 1: This case involves a 16-month-old patient who has been admitted for over
three months for chronic respiratory failure and has to be on a vent. The patient’s
caregiver has been at bedside since the admission, often having to go to and from their
home which is over two hours away. Due to the patient needs, the caregiver has not
been able to maintain consistent employment which has significantly reduced her
income/financial resources resulting in becoming behind on their rent, electric bill and
car payment. Therefore, we are submitting for you consideration the following needs:
Rent ($3200.00), Electric ($170.00) and Car payment ($200.00). Total Ask: $3,570.00 APPROVED
Case 2: Note: This family received Baby Sharon assistance in Jan 2025. This case
involves a 2y old patient with End Stage Renal Disease which requires nightly
Peritoneal Dialysis performed by their caregivers. The patient also has monthly care
plan appointments in the Nephrology Clinic which can last between 2-3 hours each visit
and occasional inpatient admissions to address issues related to Chronic Kidney
Disease. The patient will remain on night treatment until they are eligible for a live-
saving kidney transplant. Managing the patient’s treatment and appointment needs can
be very difficult for two working caregivers who also have other children at home. Since
the patient is not school age, they are now cared for during the day by a grandparent.
However, the grandparent has recently developed some health issues which have
caused one of the caregivers to have to miss work. Their job does not provide paid time
off, so this has led to a reduction in income which is around $1,600. Due to this loss,
they have fallen behind on their rent. Therefore, we are submitting for your
consideration a request for $1425.00 for rent . Total ask: $1425.00 APPROVED
Case 3: This case involves a teenage patient who was born with tricuspid atresia. Pt
had a Fontan procedure at 3yrs of age. Per physician this is a catastrophic condition
due to the type of procedure as well as the patient having three cardiac surgeries, many
cardiac caths. Survival without a heart transplant is about 70% by age 20. The patient
is going to college in the fall and there is concern for syncopal episodes which happen
about twice a month. They are looking at getting a service dog trained to be able to
recognize when the patient is going to have a syncopal episode to assist with lying
down and then getting under legs to help with recovery. They were told the total cost of training would be $10,000 and are asking for any assistance in covering in this large expense. Total ask: $5,000 APPROVED
