Case 1:  This case involves nearly one-month-old patient (twin) that was admitted recently from UAMS.  During patient’s time in the hospital, they have experienced many difficulties in their care.  Patient’s twin was discharged a while ago but patient still requires to be hospitalized. Patient’s caregiver has completed CNA classes and will be starting employment soon.  However, for the past two months caregiver fell on hard times due to transportation repair needs.  The repairs were paid out of pocket which put family behind on rent.  Therefore, we are submitting for your consideration a total ask of $784.62 towards rent.

The committee supported this request for $784.62.

Case 2:  This case involves twins born at 25 weeks gestation, who were transferred to ACH NICU.  Issues for the patients include extreme prematurity, respiratory failure as well as other conditions.  Since admissions, both patients have been diagnosed with additional issues including seizures, apnea along with other and remain on ventilators.  Due to their numerous conditions, both were be hospitalized for several more months and require additional procedure/surgeries.  The caregiver was employed prior to the patients’ arrival and since has been taking freelance work when available.  Caregiver is actively seeking out employment in order to pay bills but has been bedside since admission.  Caregiver also has own medical issues that limits their ability to work.  Additionally, the grandmother lives in the home and is disabled.  She has been able to provide some financial support when she can.  Within the last weeks, the family’s lease ended so they had to find new housing.  This required them to pay a deposit plus first and last month’s rent. This caused additional financial stress to their current situation so they are seeking assistance.  Therefore, we are submitting for your consideration a total ask of $3764 to assist with rent needs. 

The committee agreed to support 2 months rent for $1882.

Case 3:  This case involves a teenage patient admitted for a gunshot wound, which has left them a quadriplegic.  The family has been at bedside since the event which has kept the father from being able to go to work.  Caregiver is currently on FMLA but it is unpaid and they are learning to adapt to their new normal now.  This understandably has caused a financial strain for the family including needing to maintain housing.  This financial assistance will help ensure the family is able to retain their housing for another month.   Therefore we are submitting for your consideration a total ask of $718.50 towards rent.

The committee agreed to support this request 0f $718.50.

Case 4:  This case involves a 3 month old with a congenital heart condition.  Prior to patient’s birth, mom was employed but has since become a full-time caregiver at bedside for patient.  Pt’s father has become the sole financial support for family.  Unfortunately, patient’s father sustained a serious injury and is now unable to work for at least one month for healing and physical therapy.   These situations have created a significant financial burden on the family and they have begun to fall behind on rent, utilities, and car payments.  Family has received a notice of eviction if they are unable to start making payments, and are at risk of losing their care, which is the only method of reliable transportation.  Therefore, we are submitting for your consideration that following assistance:  Rent ($1,610), Auto payment ($800) and phone bill ($198.68).  Total ask:  $2,608.68

The committee agreed go support $1605 for one month rent and the auto payment.

Case 5:  This case involves a newborn admitted to CVICU and had open-heart surgery.  Pt was diagnosed with a congenital heart condition that parents were not expecting and naturally has caused undue stress on whole family.  Patient has two other siblings so family has to travel back and forth from Northeast Arkansas.  Mom is a full-time caregiver and dad is the primary financial support whose weekly income is under $700 a week.  Due to the hospitalization’s, dad’s income is not able to make end meet.  The family is already behind on rent and making payments towards utilities/car payments and phone. We are therefore submitting for your consideration the following assistance:  Rent ($2,800), Gas ($121.70) Phone ($108.89) Car Payments ($130.39 and $131.98).  Any amount will help the family get caught up enough would greatly be appreciated.    Total ask:  $3,292.96

The committee agreed to support $1530.39 for one month rent and one car payment.

Case 6:  This case involves a postnatal diagnosed hypo plastic left heart syndrome patient.  Patient had open-heart surgery and with great sadness recently passed away due to complications.  Patient’s family has no current income as they have been at patient’s bedside since admission.  Family received assistance for rent previous months from another foundation.  However, they are at risk for eviction for May.  Therefore we are submitting for your consideration a financial request in the amount of $500.00 towards rent so family can return home, settle in after the loss of their child and begin to find employment and get caught up on finances.   Total ask:  $500

The committee supported this request of $500.

Case 7:   This case involves a patient with sickle cell disease and asthma.  Unfortunately, patient acquired a serious infection that led to a sepsis hospitalization.  Patient is from a single parent household with two other siblings.  Caregiver was at risk for losing their job due to not being eligible for FMLA.  Patient’s birthday is coming up and they are sad to not be able to buy the patient presents again this year.  Their electric company sent them a shut off notice for May but were able to get a medical extension, as the medications require refrigeration.  Therefore, we are submitting for your consideration the sum of $633.57 towards their electric bill.  This would help family catch up and provide a fresh start as they get back on their feet.   Total ask $633.57

The committee supported this request of $633.57.

Case 8: This case involves a newborn transferred to ACH for treatment of respiratory distress. The patient has since been diagnosed with multiple conditions including ventricular septal defect and pulmonary hypertension.  Patient remains on oxygen support and is receiving all feeds from a nasogastric tube.  Patient is anticipated to need surgery as well.  Patient’s mother has remained at bedside and the discharge date is not anticipated to be for several more weeks.  Patient’s family has experienced financial difficulties since fall when father became injured on the job.  Father has been working part time for the past few months but are struggling to pay bills on a limited income.  Patient’s mother was working until she was placed on gestational bedrest.  Mother plans to return to work once patient is discharged home.  Due to these difficult situations, we are requesting for your consideration the following total:  $1,700 towards rent.

The committee agreed to support $1000 to assist with rent.

Case 9: This case involves a 5yr chronically ill patient.  Patient has been admitted for over a month and there is not a current timeframe for discharge.  Caregiver has another child at home and therefore has been traveling back in and forth, as they are unable to get childcare.  This admissions and traveling costs have placed a strain on family’s finances.  Family is seeking help with utility bills (Electric $168.73), Gas ($189.32), Water ($114.82) and Internet ($288.55) to avoid disconnection.  Therefore we are submitting for your consideration a total ask of $761.42

The committee supported the request of $761.42.

Case 10: This case involves a 5 month old that was born at 24 weeks. Patient has been in NICU since Feb. Patient currently suffers from multiple conditions including Chronic Lung disease of prematurity; Extreme immaturity of newborn Hypospadias, Retinopathy of prematurity, Tracheobronchomalacia.  Patient is currently admitted and awaiting pulmonary consult, currently working on their nutrition and growth. Additionally patient’s twin passed which has created financial and emotional fragility in family. Patient’s father is primary source of income in the household. Mother is unable to work, as she is needed at bedside for purposes of comfort, support, caregiving, and consent.  Family recently moved into new apartment.  Patient’s medical conditions have placed family in financial strains including making car payment.  There is not a discharge date as patient is struggling to meet milestone and this process may take weeks or months.  Mother does not receive food stamps. Patient’s mother is WIC beneficiary herself and patient which will be once they discharge.  Therefore, we are submitting for your consideration a request to help with their car payment of $534.80.  Total Ask: $534.80.

The committee supported the request of $534.80. 

Case 11: This case involves a 2-month-old patient admitted to ACH shortly after birth for several conditions including heart issues and complications with preterm gestation.  This required patient to be admitted to NICU and CVICU.  Patient has three other siblings at home.  The father works full time and mom works part time.  Mother has limited availability to work due to family not having their own car.  Father has been loaned a car to use at this time.  Due to father’s work schedule and lack of transportation, parents have not been able to visit patient often.   Having patient going between each unit for this extended time has been stressful on the family in general but also putting a financial strain on them as well.  Should family become further behind on their rent, the property owner may evict or seek legal action.  In that event, the family would not have a safe home for patient to discharge.  They also are behind on their utility bills (electric, gas, and water).   Should the financial request be approved the family would have the opportunity to be caught up and possibly focus on saving to purchase a vehicle and allow both parents to work full-time.  Therefore, we are requesting for your consideration a total ask of $3044.27 for rent/utilities.

The committee agreed to support $2584.

Case 12:  This case involves a 3 y/o, complex care patient, followed by SWLR Clinic as well as Neurology for multiple conditions including Heterotaxy syndrome with asplenia, congenital asplenia and seizures, complex congenital heart defect. Due to the complex needs/care for the patient, the family has suffered financial setbacks which as impacted their ability to make their monthly rent. Family is seeking assistance of $900.00 towards rent, which would prevent them from being evicted.  Total ask:  $900.00
